About this form

We just need a few details in order to configure your assessment experience correctly, and this form will gather the information we need. Please take the 10 minutes or so needed to complete this form and submit your answers to us. Please note all questions are required.

About Assesees and Raters

There are two groups of people about whom we require information:

Assesees. These are the individuals who will be assessed.
Raters. These are the individuals who will be providing feedback on each Asessee.

The next group of questions are about your Assesees. (We'll ask about Raters on the next page.).

How do you wish to enroll your Assesees? *

We provide you with a URL where you can send them to enroll themselves

Raters

Each Assesee will require a group of colleagues to provide the 360 feedback. We recommend a minimum of 6, and a maximum of 15 Raters for each Assesee.

We will typically contact your Assesees to get Rater details 2 business days after you submit this form, but if you'd like us to wait a while so you can give your Assesees a heads-up, then choose a different date using the date picker above.

Contact your Assesees to ask for their Raters either in 2 business days from now, or on the date of your choosing.

Check back with you in a week from now to see if you have sent the Rater submission form and instructions to your Assesees.
